The Oklahoma State Department of Health issues certified copies of birth certificates for births that took place in Le Flore County. Non-sealed birth records that occurred over 125 years from the date of request are open to the public and require no proof of eligibility. However, a requestor must still pay $15 and submit a completed application and a photo ID. A Requestor of sealed birth records ( e.g., adoptions), which occurred over 125 years from the date of request, must provide a certified copy of a court order, to access such birth records.
